FT232H Module not detecting

Hello,

I have made a PCB module with FT232H using reference of UM232H, schematic is same as UM232H.

I followed procedure mentioned in “AN_396 FTDI Drivers Installation Guide for Windows 10” after that I connected module to my PC it detected in Device manager as USB serial convertor also detected in in FT_Prog. All seems good but only for 3 days. After 3 days suddenly module failed to detect in device manager and in FT_Prog. So I assembled 2nd PCB module with same design as previous but after few days it also failed to detect. Even I didn’t connect it to any other device but still problem occurs.

All Voltage levels are as per datasheet of FT232H. Need suggestion on this.

Hello,

Would you be able to provide the exact schematic you used in your design?
And are you noticing any signs of burning on the IC after it fails to enumerate?

1) Please find enclosed schematic .pdf file.
2) After connecting to USB PC port, IC does generate 3.3V.
3) Crystal frequency (12 MHz) which was appearing previously has stooped appearing even after crystal change. So inverter inside IC may have failed along with IC itself.

Thank you for the schematic, after a quick review I cannot see any glaring issues.

Can you try removing D2/D3 on the USB data lines and let me know if the device enumerates?
Are either R7 or R5 populated? and just to clarify there are no physical signs of damage to the IC?

1) Tried removing D2, D3, R7 or R5 but off no avail.
2) You must have had large experience using this IC, I request you to suggest some testing technique to come out off present impasse other than consolation statement of "I cannot see any glaring issues".
